Summary of project

1 form build (post purchase questionnaire with 5 opinion scale questions and 1 long text question) – 3 credits
- Includes:
  - 2 rounds of design amends
- Does not include
  - Additional section logic blocks or advanced personalisation
- Things to note:
  - Responses are stored and can be exported as a .csv file
  - B there is a character limit of 1600 where users who receive a notification that the form has been submitted are added to the form. If we assume that emails are 30 characters in length, then Essentra could have just over 50 email addresses receiving notifications.

1 thank you message added into the confirmation block once after submission
- A thank you message – 0 credits
  - The message is configured in the Confirmation block of the questionnaire itself using copy supplied by Essentra and is included in the post purchase questionnaire build price
  - A link to the homepage will be included (or to any page on the website supplied by the client)

1 email campaign build containing a link to the post purchase questionnaire above – 3 credits
- Includes:
  - 2 rounds of design amends
- Does not include:
  - Configuration of the trigger used to send this email, either via a program or via the API
  - Additional dynamic content blocks or advanced personalisation

Professional Services Terms

The work described hereunder must be redeemed within 12 months from purchase, on a date and time to be agreed with dotdigital. Unless due to the availability of Engagement Cloud, any failure to instruct dotdigital to commence or agree the work described (in whole or in part) will result in forfeiture of the entirety of the fee and no credits or refunds, pro rata or otherwise, will be provided.
